Obstetrics & Gynecology - Surgical Procedures

Gynecological surgery can include any of the following treatments: ovarian surgery, hysterectomy, anterior or posterior repairs, dilatation and curettage, and fallopian tube surgery. The most commonly preformed type of gynecological surgery is the hysterectomy. This may be needed to help with endometriosis, or cancer, and involves removal of the uterus and sometimes the fallopian tubes.

Dilatation and curettage, sometimes called a D&C, is performed when there is has been a miscarriage, to help with uterine cancer, to help with heavy menstrual bleeding, and to treat uterine polyps. The procedure requires the cervix to be opened and the curette scrapes the lining of the uterus. While this procedure is quite effective, there are times when too much tissue is removed causing infections of the fallopian tubes or the uterus, as well as implantation issues with pregnancies.

Anterior and posterior repairs are required when there is a thinning or sagging of the vaginal walls or tissues. This happens from damage during the birth process, and is remedied by removing a section of the vagina. The incisions are then stitched back together to make a tighter area. If this surgical procedure is done incorrectly, damage to the urethra or bladder can result. If this happens, more surgery may be needed, or the patient will have to drain the bladder with a catheter until the damage is repaired.

Another type of gynecological surgery that is common is the tubal ligation. This is done to prevent future pregnancies, and is considered to be effective. That being said, there is a small change it will not work completely, resulting in pregnancies that are unplanned. The legal issues that are associated with this procedure usually related to patients not being made aware of this risk, and not supplying informed consent. Tubal ligations are completed by removing a section of the fallopian tube, cutting the ends, and then tying, clipping, or burning them back together.

Ovarian surgery may be needed for several reasons, such as to remove cancerous growths, to handle endometriosis, or to remove cysts. Not all cases will require the complete removal of the ovaries, and there are often times when just the cyst, tumor, or endometrial spot can be removed while preserving the remainder of the ovary.
